Psoriasis has no one known cause, however it has been identified as an immune response in the body with a link to genetics.
Not everyone genetically inherits the psoriasis gene, will experience psoriasis in their lifetime. However environmental stressors are often the triggers in those who are susceptible.
Once triggered psoriasis causes the immune system to speed up skin cell production, causing red, scaly plaques and patches on the skin. The most common type of psoriasis cause defined, raised, red plaques with silver scales, most commonly seen on the knees, elbows, scalp and lower back. They can unfortunately crack or bleed causing pain.
Environmental triggers of psoriasis include bacterial infections eg: streptococcal throat, emotional stress eg: bereavement, exam stress, injury to the skin and some medications may cause the first, and hopefully only episode of psoriasis. Lifestyle factors can also worsen the skin condition eg: smoking, drinking alcohol.
There are a few different types of psoriasis:
1. Plaque Psoriasis: as mentioned above, defined silver plaques.
2. Guttate Psoriasis: usually sudden onset with small red, teardrop patches. Usually after a throat infection or exam stress.
3. Flexural Psoriasis: occurs in the skinfolds, groin, armpits. These plaques are red, smooth and shiny.
4. Genital Psoriasis: similar to flexural psoriasis occurring around the genitalia.
5. Scalp Psoriasis: this may be the only area affected and presents like dandruff, with some red scales around the hairline and ears.
6. Nail Psoriasis: thickening of the nails usually associated with a diagnosis of psoriatic arthritis.
7. Palmo- Plantar Pustulosis: presents only on palms of hands and soles of the feet with small liquid filled blisters. Can become red and scaly.
While there is currently no cure, psoriasis can be effectively managed with the right treatment plan.

Where psoriasis is severe, widespread, or requires specialist treatment, patients are referred to an appropriate consultant specialist in line with national guidelines. Dr Sylvia continues to support patients before and after referral, where appropriate, as part of their ongoing care in primary care.
